Heatstroke pathophysiology: the energy depletion model.

چکیده

This symposium focuses on exertional heatstroke, with emphasis on predisposing factors, clinical observations, diagnosis, treatment and recovery. The serious challenge to cardiovascular stability presented by exercise hyperthermia and some aspects of regulatory failure was reviewed in the introduction. The intent of this review, to understand how heat stress is translated into heat strain at the cellular level, is not to downplay the seriousness of the systemic condition. If we are successful, this may stimulate further interest on the impact of heat on the cell as a model for other factors which alter membrane integrity and permeability, lead to new experimental paradigms, and improve the diagnosis and treatment of other disorders such as toxic, hypovolemic, or ischemic shock. This review may also stimulate interesting research regarding more subtle threats to homeostasis such as chronic exercise, hypohydration and thirst, ion imbalance, and sleep deprivation. Since the original article on heat stroke pathophysiology was published (39), we have discussed the relationship of this concept to carbohydrate metabolism (40), thirst (41), and the cellular aspects of heat illness treatment (99). This article will attempt to integrate and extend some of those ideas.

Heating Causes a Rise in the Concentration of Intracellular Sodium in Human Cells in Vitro

A rise in intracellular sodium ion concentration ([Na+]i) was predicted by Hubbard in his "energy depletion mechanism" of heatstroke [ l ] but such rises have not been clearly shown. In this study, we have found rises in [Na+]i at elevated temperatures in isolated human squamous epithelial cells, by measuring changes in the fluorescence of a Na+-sensitive fluorophore which had been introduced i...

Heatstroke: thermoregulation, pathophysiology, and predisposing factors.

Heatstroke is a common veterinary emergency that, depending on the severity of injury, can progress to a life-threatening condition. Heatstroke can be classic (nonexertional) or exertional. Classic heatstroke develops when the body is exposed to high external temperatures, whereas exertional heatstroke is caused by strenuous exercise. Thermoregulation is the intrinsic ability of the body to mai...

Heatstroke is a complex disease process that, in its most severe form, can result in multiorgan dysfunction and death. Heatstroke stems from the failure of the body’s thermoregulatory mechanisms, resulting in cellular damage and death. The organ systems most commonly affected in this disease process include the gastrointestinal tract and the coagulation, renal, cardiac, pulmonary, and central n...

Microvascular injury, thrombosis, inflammation, and apoptosis in the pathogenesis of heatstroke: a study in baboon model.

OBJECTIVE Severe heatstroke is a leading cause of morbidity and mortality during heat waves. The pathogenesis of tissue injury, organ failure, and death in heatstroke is not well understood. METHODS AND RESULT We investigated the pathways of heatstroke-induced tissue injury and cell death in anesthetized baboons (Papio hamadyras) subjected to environmental heat stress until core temperature a...
